Joe Perches 3c355863fb testmmiotrace.c: Add and use pr_fmt(fmt) ...

- Add #define pr_fmt(fmt) KBUILD_MODNAME ": " fmt.
- Strip MODULE_NAME from pr_<level>s.
- Remove MODULE_NAME definition.
