Open call is ignoring the return code from reset call and can
potentially continue even though reset call failed.
If reset_required module parameter is set, this patch is going
to validate the return code and will abort open if reset fails.

Even when the adapter is configured for CEC 1.4, we still check and
use the CEC 2.0 parts of struct cec_log_addrs. Although these aren't
used in CEC messages, the information contained in them is still of
use in the CEC framework itself, so keep this information.
Also zero the unused trailing features[] data and unused logical address
data so the contents isn't random data.

The code was allowing platform devices to be used without a supporting
VFIO reset driver. The hardware can be left in some inconsistent state
after a guest machine abort.
The reset driver will put the hardware back to safe state and disable
interrupts before returning the control back to the host machine.
Adding a new reset_required kernel module option to platform VFIO drivers.
The default value is true for the DT and ACPI based drivers.
The reset requirement value for AMBA drivers is set to false and is
unchangeable to maintain the existing functionality.
New requirements are:
1. A reset function needs to be implemented by the corresponding driver
via DT/ACPI.
2. The reset function needs to be discovered via DT/ACPI.
The probe of the driver will fail if any of the above conditions are
not satisfied.

The device tree code checks for the presence of a reset driver and calls
the of_reset function pointer by looking up the reset driver as a module.
ACPI defines _RST method to perform device level reset. After the _RST
method is executed, the OS can resume using the device. _RST method is
expected to stop DMA transfers and IRQs.
This patch introduces two functions as vfio_platform_acpi_has_reset and
vfio_platform_acpi_call_reset. The has reset method is used to declare
reset capability via the ioctl flag VFIO_DEVICE_FLAGS_RESET. The call
reset function is used to execute the _RST ACPI method.

The code is using the compatible DT string to associate a reset driver
with the actual device itself. The compatible string does not exist on
ACPI based systems. HID is the unique identifier for a device driver
instead.

The main message loop checks if the physical address was valid, and if
not it is assumed that the adapter had been unconfigured.
However, this check is no longer correct, instead it should check
that both adap->is_configured and adap->is_configuring are false.

The size of the transmit queue was unlimited, which meant that
in non-blocking mode you could flood the CEC adapter with messages
to be transmitted.
Limit this to 18 messages.
Also print the number of pending transmits and the timeout value
in the status debugfs file.

Ensure that the unused part of the msg array is zeroed. This is
required by CEC 2.0 when receiving shorter messages than expected.
In that case the remaining bytes are assumed to be 0.
There are no such CEC messages yet, but it is required to be future proof.
And since we're doing it for received messages, do it for transmitted
messages as well. It's unambiguous this way.

The filehandle was set to NULL when in non-blocking mode or when
no reply is needed.
This is wrong: the filehandle is needed in non-blocking mode to ensure
that the result of the transmit can be obtained through CEC_RECEIVE.
And the 'reply' check was also incorrect since it should have checked the
timeout field (the reply can be 0). In any case, when in blocking mode
there is no need to set the fh to NULL either.

Stepping with PSTATE.D=1 is bad news. The step won't generate a debug
exception and we'll likely walk off into random data structures. This
should never happen, but when it does, it's a PITA to debug. Add a
WARN_ON to shout if we realise this is about to take place.

Clearing PSTATE.D is one of the requirements for generating a debug
exception. The arm64 booting protocol requires that PSTATE.D is set,
since many of the debug registers (for example, the hw_breakpoint
registers) are UNKNOWN out of reset and could potentially generate
spurious, fatal debug exceptions in early boot code if PSTATE.D was
clear. Once the debug registers have been safely initialised, PSTATE.D
is cleared, however this is currently broken for two reasons:
(1) The boot CPU clears PSTATE.D in a postcore_initcall and secondary
CPUs clear PSTATE.D in secondary_start_kernel. Since the initcall
runs after SMP (and the scheduler) have been initialised, there is
no guarantee that it is actually running on the boot CPU. In this
case, the boot CPU is left with PSTATE.D set and is not capable of
generating debug exceptions.
(2) In a preemptible kernel, we may explicitly schedule on the IRQ
return path to EL1. If an IRQ occurs with PSTATE.D set in the idle
thread, then we may schedule the kthread_init thread, run the
postcore_initcall to clear PSTATE.D and then context switch back
to the idle thread before returning from the IRQ. The exception
return path will then restore PSTATE.D from the stack, and set it
again.
This patch fixes the problem by moving the clearing of PSTATE.D earlier
to proc.S. This has the desirable effect of clearing it in one place for
all CPUs, long before we have to worry about the scheduler or any
exception handling. We ensure that the previous reset of MDSCR_EL1 has
completed before unmasking the exception, so that any spurious
exceptions resulting from UNKNOWN debug registers are not generated.
Without this patch applied, the kprobes selftests have been seen to fail
under KVM, where we end up attempting to step the OOL instruction buffer
with PSTATE.D set and therefore fail to complete the step.

raid_io_hints() was retrieving the number of data stripes used for the
calculation of io_opt from struct r5conf, which is not defined for raid0
mappings.
Base the calculation on the in-core raid_set structure instead.
Also, adjust to use to_bytes() for the sector -> bytes conversion
throughout.

Sphinx is really evil when an older version finds an extra
attribute for the :toctree: tag: it simply ignores everything
and produce documents without any chapter inside!
As we're now using tags available only on Sphinx 1.4.x, we
need to use some creative ways to add a title before the
table of contents. Do that by using a css class.

The TCG standard startup sequence (get timeouts, tpm startup, etc) for
TPM and TPM2 chips is being open coded in many drivers, move it into
the core code.
tpm_tis and tpm_crb are used as the basis for the core code
implementation and the easy drivers are converted. In the process
several small drivers bugs relating to error handling this flow
are fixed.
For now the flag TPM_OPS_AUTO_STARTUP is optional to allow a staged
driver roll out, but ultimately all drivers should use this flow and
the flag removed. Some drivers still do not implement the startup
sequence at all and will need to be tested with it enabled.

If devm_add_action() fails we are explicitly calling put_device() to
free the resources allocated. Lets use the helper
devm_add_action_or_reset() and return directly in case of error, as we
know that the cleanup function has been already called by the helper if
there was any error.

Some chips incorrectly support partial reads from TPM_STS register
at non-zero offsets. Read the entire 32-bits register instead of
making two 8-bit reads to support such devices and reduce the number
of bus transactions when obtaining the burstcount from TPM_STS.

The result must be converted from BE byte order, which is used by the
TPM2 protocol. This has not popped out because tpm2_get_tpm_pt() has
been only used for probing.

We currently define OBJCOPYFLAGS in the top-level arm64 Makefile, and
thus these flags will be passed to all uses of objcopy, kernel-wide, for
which they are not explicitly overridden. The flags we set are intended
for converting vmlinux (and ELF) into Image (a raw binary), and thus the
flags chosen are problematic for some other uses which do not expect a
raw binary result, e.g. the upcoming lkdtm rodata test:

This patch localises the objcopy flags such that they are only used for

The pre-handler of this special 'trampoline' kprobe executes the return
probe handler functions and restores original return address in ELR_EL1.
This way the saved pt_regs still hold the original register context to be
carried back to the probed kernel function.

The trampoline code is used by kretprobes to capture a return from a probed
function. This is done by saving the registers, calling the handler, and
restoring the registers. The code then returns to the original saved caller
return address. It is necessary to do this directly instead of using a
software breakpoint because the code used in processing that breakpoint
could itself be kprobe'd and cause a problematic reentry into the debug
exception handler.

Kprobes needs simulation of instructions that cannot be stepped
from a different memory location, e.g.: those instructions
that uses PC-relative addressing. In simulation, the behaviour
of the instruction is implemented using a copy of pt_regs.
The following instruction categories are simulated:
- All branching instructions(conditional, register, and immediate)
- Literal access instructions(load-literal, adr/adrp)
Conditional execution is limited to branching instructions in
ARM v8. If conditions at PSTATE do not match the condition fields
of opcode, the instruction is effectively NOP.

Add support for basic kernel probes(kprobes) and jump probes
(jprobes) for ARM64.
Kprobes utilizes software breakpoint and single step debug
exceptions supported on ARM v8.
A software breakpoint is placed at the probe address to trap the
kernel execution into the kprobe handler.
ARM v8 supports enabling single stepping before the break exception
return (ERET), with next PC in exception return address (ELR_EL1). The
kprobe handler prepares an executable memory slot for out-of-line
execution with a copy of the original instruction being probed, and
enables single stepping. The PC is set to the out-of-line slot address
before the ERET. With this scheme, the instruction is executed with the
exact same register context except for the PC (and DAIF) registers.
Debug mask (PSTATE.D) is enabled only when single stepping a recursive
kprobe, e.g.: during kprobes reenter so that probed instruction can be
single stepped within the kprobe handler -exception- context.
The recursion depth of kprobe is always 2, i.e. upon probe re-entry,
any further re-entry is prevented by not calling handlers and the case
counted as a missed kprobe).
Single stepping from the x-o-l slot has a drawback for PC-relative accesses
like branching and symbolic literals access as the offset from the new PC
(slot address) may not be ensured to fit in the immediate value of
the opcode. Such instructions need simulation, so reject
probing them.
Instructions generating exceptions or cpu mode change are rejected
for probing.
Exclusive load/store instructions are rejected too. Additionally, the
code is checked to see if it is inside an exclusive load/store sequence
(code from Pratyush).
System instructions are mostly enabled for stepping, except MSR/MRS
accesses to "DAIF" flags in PSTATE, which are not safe for
probing.
This also changes arch/arm64/include/asm/ptrace.h to use
include/asm-generic/ptrace.h.

Certain instructions are hard to execute correctly out-of-line (as in
kprobes). Test functions are added to insn.[hc] to identify these. The
instructions include any that use PC-relative addressing, change the PC,
or change interrupt masking. For efficiency and simplicity test
functions are also added for small collections of related instructions.

When disabling tethering after having enabled it, the ath6kl driver does
not set the IFF_LOWER_UP flag to 0 (carrier off) on the wlan interface.
The upper layers (eg. connman) are not notified of the tethering mode
status change. So, tethering can not be activated anymore.
This patch adds a netif_carrier_off() call when stopping AP mode to fix
the problem.

One minute after a successful connection, the kernel checks if the
frequency and the channel width are well configured for the country we
are in. (regulatory domain)
ath6kl driver was setting the NL80211_CHAN_HT20 channel without checking
for the HT capabilities. (we should have NL80211_CHAN_NO_HT in our
case because the firmware did not support HT)
This patch adds a check on ht_cap.ht_supported in order to create the
channel corresponding to the firmware capabilities.

When enabling WLAN tethering, a new AP is visible and a STA could
connect to it. When the STA tries to authenticate to the newly created
AP, the WPA authentication mechanism is stuck in the 1/4 msg of 4-Way
Handshake.
In ath6kl_rx(), the ath6kl_find_sta() function is looking for the
h_source field of the Ethernet frame header received by the STA. The
datap pointer that points to the Ethernet frame header is incorrect,
and was pointing at the wrong offset in the buffer.
This commit adds a pad_before_data_start offset to set the datap pointer
to the Ethernet frame header. datap->h_source parameter is now really
pointing to the source ethernet address and the authentication process
can continue.
