0f2aa8caea usb: musb: ux500: Add check for NULL board data
Dan Carpenter's automatic Smatch checker found an anomaly in the ux500
MUSB driver, whereby board data was checked before use in all but one
occasion. It is believed that it needs to be checked every time.

Smatch complaint:

         error: we previously assumed 'data' could be null (see line 313)

9473e8cfc7 usb: phy: fix build breakage
Commit 94ae9843 (usb: phy: rename all phy drivers to phy-$name-usb.c)
renamed drivers/usb/phy/otg_fsm.h to drivers/usb/phy/phy-fsm-usb.h
but changed drivers/usb/phy/phy-fsm-usb.c to include not existing
"phy-otg-fsm.h" instead of new "phy-fsm-usb.h". This breaks building:
  ...
  drivers/usb/phy/phy-fsm-usb.c:32:25: fatal error: phy-otg-fsm.h: No such file or directory
  compilation terminated.
  make[3]: *** [drivers/usb/phy/phy-fsm-usb.o] Error 1

This commit also missed to modify drivers/usb/phy/phy-fsl-usb.h
to include new "phy-fsm-usb.h" instead of "otg_fsm.h" resulting
in another build breakage:

b0eb57cb97 VMXNET3: Add support for virtual IOMMU
This patch adds support for virtual IOMMU to the vmxnet3 module.  We
switch to DMA consistent mappings for anything we pass to the device.
There were a few places where we already did this, but using pci_blah();
these have been fixed to use dma_blah(), along with all new occurrences
where we've replaced kmalloc() and friends.

Also fix two small bugs:
1) use after free of rq->buf_info in vmxnet3_rq_destroy()
2) a cpu_to_le32() that should have been a cpu_to_le64()

bea5098848 be2net: Fix be_cmd_if_create() to use MBOX if MCCQ is not created
Currently the IF_CREATE FW cmd is issued only *after* MCCQ is created as
it was coded to only use MCCQ. By fixing this, cmd_if_create() can be
called before MCCQ is created and the same routine for VF provisioning
can be called after.
This allows for consolidating all the queue create routines by moving
the be_cmd_if_create() call above all queue create calls in be_setup().

92bf14abf7 be2net: refactor be_get_resources() code
1) use be_resources{} struct to query/store HW resource limits
2) The HW queue/resource limits for BE2/BE3 chips are mostly called out
   in driver as constants.  Code to handle this is scattered across various
   places in be_setup(). Consolidate this code into BEx_get_resources().
   For Lancer-R, Skyhawk-R, these limits are queried from FW.

during Tx.  This fix is small, tested by the bug reported and isolated
to ath9k_htc.

Stanislaw Gruszka reverts a recent iwl4965 change that broke rfkill
notification to user space.

b1dcdc68b1 net: tcp_probe: allow more advanced ingress filtering by mark
Currently, the tcp_probe snooper can either filter packets by a given
port (handed to the module via module parameter e.g. port=80) or lets
all TCP traffic pass (port=0, default). When a port is specified, the
port number is tested against the sk's source/destination port. Thus,
if one of them matches, the information will be further processed for
the log.

As this is quite limited, allow for more advanced filtering possibilities
which can facilitate debugging/analysis with the help of the tcp_probe
snooper. Therefore, similarly as added to BPF machine in commit 7e75f93e
("pkt_sched: ingress socket filter by mark"), add the possibility to
use skb->mark as a filter.

If the mark is not being used otherwise, this allows ingress filtering
by flow (e.g. in order to track updates from only a single flow, or a
subset of all flows for a given port) and other things such as dynamic
logging and reconfiguration without removing/re-inserting the tcp_probe
module, etc. Simple example:

  insmod net/ipv4/tcp_probe.ko fwmark=8888 full=1
  ...
  iptables -A INPUT -i eth4 -t mangle -p tcp --dport 22 \
           --sport 60952 -j MARK --set-mark 8888
  [... sampling interval ...]
  iptables -D INPUT -i eth4 -t mangle -p tcp --dport 22 \
           --sport 60952 -j MARK --set-mark 8888

The current option to filter by a given port is still being preserved. A
similar approach could be done for the sctp_probe module as a follow-up.

ce26bd23d4 USB: gadget: audit sysfs attribute permissions
Convert all USB gadget sysfs attributes to use the _RO or _RW variants,
to make them easier to audit and ensure that the permissions are
correct.

Note, two are left using the DEVICE_ATTR() macro, as there is no
DEVICE_ATTR_WO() in Linus's tree, that will happen after 3.12-rc1 is
out, a follow-on patch will be sent then.

ee7289bfad ceph: allow sync_read/write return partial successed size of read/write.
For sync_read/write, it may do multi stripe operations.If one of those
met erro, we return the former successed size rather than a error value.
There is a exception for write-operation met -EOLDSNAPC.If this occur,we
retry the whole write again.

2ae847a1b1 usb: musb: am335x: add second port to beagle bone
So I assumed that Beagle bone has only one USB port in host mode because
the micro USB connector had an USB-UART there. I was wrong a little. The
second port runs on host mode, but the micro USB plug is connected to an
internal HUB with two ports: one to the USB-UART and one to musb
instance one.
For that reason, this patch enables both ports: the primary in device
mode only and the second in host mode only.

781f179830 usb: musb: am335x-evm: Do not remove the session bit HOST-only mode
This is what I observe:
On the first connect, the musb starts with DEVCTL.Session set. On
disconnect, musb_core calls try_idle. That functions removes the Session
bit signalizing that the session is over (something that only in OTG is
required). A new device, that is plugged, is no longer recognized.
I've setup a timer and checked the DEVCTL register and I haven't seen a
change in VBus and I saw the B-Device bit set. After setting the IDDIG
into A mode and forcing the device to behave like a A device, I didn't
see a change.
Neither VBUS goes to 0b11 nor does a session start request comes.
In the TI-v3.2 kernel they skip to call musb_platform_try_idle() in the
OTG_STATE_A_WAIT_BCON state while not in OTG mode.
Since the second port hast a standard A plug the patch changes the port
to run in host mode only and skips the timer which would remove
DEVCTL.Session so we can reconnect to another device later.

9dfa36218d usb: musb: dsps: do not set is_active on the first drvbus interrupt
Quite early on init there is an vbus / drvvbus interrupt comming and the
dsps code sets is_active to one. As a result we see a lot of

|musb_bus_suspend 2459: trying to suspend as a_wait_bcon while active

until a device is plugged in with pm_runtime enabled in the kernel.
After checking davinci, am35, da8xx I noticed that dsps is actually the
only one doing this.
So remove it and we won't flooded with mesages and the idle port can be
suspended.

This relfects the code and dts requires changes due to recent .dts
binding updates:
- use mg prefix for the Metor Graphics specific attributes
- use power in mA not in mA/2 as specifed in the USB2.0 specification
- remove the child node for USB. This is driver specific on won't be
  reflected in the device tree
- use the "mentor" prefix instead of "mg".
- use "dr_mode" istead of "mg,port-mode" for the port mode. The former
  is used by a few other drivers.

3b6394b4fa usb: musb: dsps fix the typo in reg-names of the dma node
I forgot to separete the different names in the reg-names property. This
didn't cause anything to fail because the driver does not use the names
and simply relies on the order of the memory offsets in reg.
This patch fixes this in case it is used later.

9047428894 usb: musb: only remove host/udc if it has been added
musb_shutdown() removes always USB host and device.
musb_init_controller() adds host and device depending on port_mode. If
port mode is set to HOST then the removal of UDC leads only to:
|(NULL device *): gadget not registered.
and nothing else happens. If port mode is set to DEVICE and we remove
the host then we oops in usb_remove_hcd().
This patch ensures that we only remove host in OTG/host mode and device
only in OTG/device mode to avoid any trouble.
