As we attempt to kmalloc after calling get_pages, there is a possibility
that the shrinker may reap the pages we just acquired. To prevent this
we need to increment the pages_pin_count early, so rearrange the code
and error paths to make it so.

We shouldn't disable the trickle feed bits on Haswell. Our
documentation explicitly says the trickle feed bits of PRI_CTL and
CUR_CTL should not be programmed to 1, and the hardware engineer also
asked us to not program the SPR_CTL field to 1. Leaving the bits as 1
could cause underflows.

Systems with Intel graphics controllers set aside memory exclusively for
gfx driver use. This memory is not always marked in the E820 as
reserved or as RAM, and so is subject to overlap from E820 manipulation
later in the boot process. On some systems, MMIO space is allocated on
top, despite the efforts of the "RAM buffer" approach, which simply
rounds memory boundaries up to 64M to try to catch space that may decode
as RAM and so is not suitable for MMIO.

RCS flips do work on Iybridge+ so long as we can unmask the messages
through DERRMR. However, there are quite a few workarounds mentioned
regarding unmasking more than one event or triggering more than one
message through DERRMR. Those workarounds in principle prevent us from
performing pipelined flips (and asynchronous flips across multiple
planes) and equally apply to the "known good" BCS ring. Given that it
already appears to work, and also appears to work with unmasking all 3
planes at once (and queuing flips across multiple planes), be brave.

We now have more devices using ring->private than not, and they all want
the same structure. Worse, I would like to use a scratch page from
outside of intel_ringbuffer.c and so for convenience would like to reuse
ring->private. Embed the object into the struct intel_ringbuffer so that
we can keep the code clean.

Add a uImage build target for MIPS, which builds uImage.gz (a U-Boot
image of vmlinux.bin.gz), and then symlinks it to uImage. This allows
for the use of other compression algorithms in future, and is how a few
other architectures do it.
It's enabled conditionally on load-y >= 0xffffffff80000000 which
hopefully allows 64bit kernels to also work as long as the load and
entry address can be represented by the 32bit addresses in the U-Boot
image format.

The vmlinux load address and entry address is calculated in multiple
places:
- arch/mips/Makefile defines load-y from CONFIG_PHYSICAL_START (or
defined by the platform) and passes it to
arch/mips/boot/compressed/Makefile.
- arch/mips/boot/compressed/Makefile calculates kernel entry using nm.
- arch/mips/lasat/image/Makefile calculates both load and entry address
using nm.
Lets combine these in the main Makefile and then pass them as Make
parameters to each of the three boot image Makefiles (in boot/,
boot/compressed, lasat/image/). The boot/ Makefile doesn't currently use
them, but will soon need to for U-Boot image targets.
The existing load-y definition is used in preference to calculating the
load address using nm.

Merge lockref infrastructure code by me and Waiman Long.
I already merged some of the preparatory patches that didn't actually do
any semantic changes earlier, but this merges the actual _reason_ for
those preparatory patches.
The "lockref" structure is a combination "spinlock and reference count"
that allows optimized reference count accesses. In particular, it
guarantees that the reference count will be updated AS IF the spinlock
was held, but using atomic accesses that cover both the reference count
and the spinlock words, we can often do the update without actually
having to take the lock.
This allows us to avoid the nastiest cases of spinlock contention on
large machines under heavy pathname lookup loads. When updating the
dentry reference counts on a large system, we'll still end up with the
cache line bouncing around, but that's much less noticeable than
actually having to spin waiting for the lock.

The NR_CPUS_DEFAULT_1 introduced as an aid for the QEMU
platform in 72ede9b189
"[MIPS] Qemu: Fix Symmetric Uniprocessor support"
which was later removed in
302922e5f6
"[MIPS] Qemu: Remove platform."
On certain randconfigs it may happen for NR_CPUS to have an
empty value because not all SMP platforms select a suitable
NR_CPUS_DEFAULT_* value. We fix this by restoring the range
of NR_CPUS to 2..64 and drop the NR_CPUS_DEFAULT_{1,2} symbols.
The first one is no longer used and the latter is not needed since
NR_CPUS=2 is now the default value.
Fixes the following problem on a randconfig:

@detached is set under the protection of sg_index_lock. Without getting the
lock, new sfp will be added during sg removal and there is no chance for it
to be picked out. So check with sg_index_lock held in sg_add_sfp().

A race condition may happen if two threads are both trying to open the same sg
with O_EXCL simultaneously. It's possible that they both find fsds list is
empty and get_exclude(sdp) returns 0, then they both call set_exclude() and
break out from wait_event_interruptible and resume open.
Now use rwsem to protect this process. Exclusive open gets write lock and
others get read lock. The lock will be held until file descriptor is closed.
This also leads 'exclude' only a status rather than a check mark.

Commit b90ebc3d5c ("[SCSI] scsi_debug:
fix logical block provisioning support") fixed several issues with
logical block provisioning support, but it still doesn't properly fix
the cases when unmap_alignment > 0.
For example, load scsi_debug module with the following module parameters
and make all blocks mapped by filling the storage with zero.
# modprobe scsi_debug lbpu=1 unmap_alignment=1 unmap_granularity=4
# dd if=/dev/zero of=$DEV
Then, try to unmap the first unmappable blocks at lba=1, but GET LBA STATUS
unexpectedly reports that the last UNMAP has done nothing.
# sg_unmap --lba=1 --num=4 $DEV
# sg_get_lba_status --lba=1 $DEV
descriptor LBA: 0x0000000000000001 blocks: 16383 mapped
The problem is in map_index_to_lba(), which should return the first
LBA which is corresponding to a given index of provisioning map
(map_storep).

With module parameter num_parts > 0, partition table is built on the
ramdisk storage when loading the driver. Unfortunately, there is an
endianness bug in sdebug_build_parts(). So the partition table is not
correctly initialized on big-endian systems.

On any Adapter when we flash through FC/FCoE without card reset option it still
shows the Running FW version in Flashed FW version. This new interface will be
used by API to instruct the driver to update its cache versions.
