genirq: Add IRQF_NO_AUTOEN for request_irq/nmi()
[ Upstream commit cbe16f35bee6880becca6f20d2ebf6b457148552 ] Many drivers don't want interrupts enabled automatically via request_irq(). So they are handling this issue by either way of the below two: (1) irq_set_status_flags(irq, IRQ_NOAUTOEN); request_irq(dev, irq...); (2) request_irq(dev, irq...); disable_irq(irq); The code in the second way is silly and unsafe. In the small time gap between request_irq() and disable_irq(), interrupts can still come. The code in the first way is safe though it's subobtimal. Add a new IRQF_NO_AUTOEN flag which can be handed in by drivers to request_irq() and request_nmi(). It prevents the automatic enabling of the requested interrupt/nmi in the same safe way as #1 above. With that the various usage sites of #1 and #2 above can be simplified and corrected. genirq: Take the proposed affinity at face value if force==true

Although setting the affinity of an interrupt to a set of CPUs that doesn't have any online CPU is generally frowned apon, there are a few limited cases where such affinity is set from a CPUHP notifier, setting the affinity to a CPU that isn't online yet. The saving grace is that this is always done using the 'force' attribute, which gives a hint that the affinity setting can be outside of the online CPU mask and the callsite set this flag with the knowledge that the underlying interrupt controller knows to handle it. This restores the expected behaviour on Marek's system. genirq: Always limit the affinity to online CPUs

[ Fixed small conflicts due to the HK_FLAG_MANAGED_IRQ flag been renamed on upstream ] When booting with maxcpus=<small number> (or even loading a driver while most CPUs are offline), it is pretty easy to observe managed affinities containing a mix of online and offline CPUs being passed to the irqchip driver. This means that the irqchip cannot trust the affinity passed down from the core code, which is a bit annoying and requires (at least in theory) all drivers to implement some sort of affinity narrowing. In order to address this, always limit the cpumask to the set of online CPUs. The sequence of events is: setserial open("/dev/ttyXXX") request_irq() do_stuff() -> serial interrupt -> wake(irq_thread) desc->threads_active++; close() free_irq() kthread_stop(irq_thread) synchronize_irq() <- hangs because desc->threads_active != 0 The thread is created in request_irq() and woken up, but does not get on a CPU to reach the actual thread function, which would handle the pending wake-up. kthread_stop() sets the should stop condition which makes the thread immediately exit, which in turn leaves the stale threads_active count around. Vendor kernel modules may implement irq balancers, which could take irq desc lock of an irq and then based on current affinity mask or affinity hint, reconfigure the affinity of that irq. For example : For an irq, for which affinity is broken i.e. all the cpus in its affinity mask have gone offline. For such irqs, we might want to reset the affinity, when the original set of affined cpus, come back online. desc->affinity_hint can be used for figuring out the original affinity. So, the sequence for doing this becomes: desc = irq_to_desc(i); raw_spin_lock(&desc->lock); affinity = desc->affinity_hint; raw_spin_unlock(&desc->lock); irq_set_affinity_hint(i, affinity); Here, we need to release the desc lock before calling the exported api irq_set_affinity_hint(). This creates a window where, after unlocking desc lock and before calling irq_set_affinity_hint(), where this setting can race with other irq_set_affinity_hint() callers. So, export irq_do_set_affinity() symbol to provide an api, which can be called with desc lock held. genirq: Disable interrupts for force threaded handlers

With interrupt force threading all device interrupt handlers are invoked
from kernel threads. Contrary to hard interrupt context the invocation only
disables bottom halfs, but not interrupts. This was an oversight back then
because any code like this will have an issue:

spin_lock(&foo->lock);
This has been triggered with networking (NAPI vs. hrtimers) and console
drivers where printk() happens from an interrupt which interrupted the
force threaded handler.
Now people noticed and started to change the spin_lock() in the handler to
spin_lock_irqsave() which affects performance or add IRQF_NOTHREAD to the
interrupt request which in turn breaks RT.
Fix the root cause and not the symptom and disable interrupts before
invoking the force threaded handler which preserves the regular semantics
and the usefulness of the interrupt force threading as a general debugging
tool.
For not RT this is not changing much, except that during the execution of
the threaded handler interrupts are delayed until the handler
returns. Vs. scheduling and softirq processing there is no difference.
For RT kernels there is no issue.

task_work: cleanup notification modes
A previous commit changed the notification mode from true/false to an
int, allowing notify-no, notify-yes, or signal-notify. This was
backwards compatible in the sense that any existing true/false user
would translate to either 0 (on notification sent) or 1, the latter
which mapped to TWA_RESUME. TWA_SIGNAL was assigned a value of 2.
Clean this up properly, and define a proper enum for the notification
mode. Now we have:
- TWA_NONE. This is 0, same as before the original change, meaning no
notification requested.
- TWA_RESUME. This is 1, same as before the original change, meaning
that we use TIF_NOTIFY_RESUME.
- TWA_SIGNAL. This uses TIF_SIGPENDING/JOBCTL_TASK_WORK for the
notification.
Clean up all the callers, switching their 0/1/false/true to using the
appropriate TWA_* mode for notifications.

genirq/affinity: Make affinity setting if activated opt-in
John reported that on a RK3288 system the perf per CPU interrupts are all
affine to CPU0 and provided the analysis:
"It looks like what happens is that because the interrupts are not per-CPU
in the hardware, armpmu_request_irq() calls irq_force_affinity() while
the interrupt is deactivated and then request_irq() with IRQF_PERCPU |
IRQF_NOBALANCING.
Now when irq_startup() runs with IRQ_STARTUP_NORMAL, it calls
irq_setup_affinity() which returns early because IRQF_PERCPU and
IRQF_NOBALANCING are set, leaving the interrupt on its original CPU."
This was broken by the recent commit which blocked interrupt affinity
setting in hardware before activation of the interrupt. While this works in
general, it does not work for this particular case. As contrary to the
initial analysis not all interrupt chip drivers implement an activate
callback, the safe cure is to make the deferred interrupt affinity setting
at activation time opt-in.
Implement the necessary core logic and make the two irqchip implementations
for which this is required opt-in. In hindsight this would have been the
right thing to do, but ...

genirq/affinity: Handle affinity setting on inactive interrupts correctly
Setting interrupt affinity on inactive interrupts is inconsistent when
hierarchical irq domains are enabled. The core code should just store the
affinity and not call into the irq chip driver for inactive interrupts
because the chip drivers may not be in a state to handle such requests.
X86 has a hacky workaround for that but all other irq chips have not which
causes problems e.g. on GIC V3 ITS.
Instead of adding more ugly hacks all over the place, solve the problem in
the core code. If the affinity is set on an inactive interrupt then:
- Store it in the irq descriptors affinity mask
- Update the effective affinity to reflect that so user space has
a consistent view
- Don't call into the irq chip driver
This is the core equivalent of the X86 workaround and works correctly
because the affinity setting is established in the irq chip when the
interrupt is activated later on.
Note, that this is only effective when hierarchical irq domains are enabled
by the architecture. Doing it unconditionally would break legacy irq chip
implementations.
For hierarchial irq domains this works correctly as none of the drivers can
have a dependency on affinity setting in inactive state by design.
Remove the X86 workaround as it is not longer required.

The handling of notify->work did not properly maintain notify->kref in two cases: 1) where the work was already scheduled, another irq_set_affinity_locked() would get the ref and (no-op-ly) schedule the work. Thus when irq_affinity_notify() ran, it would drop the original ref but not the additional one. 2) when cancelling the (old) work in irq_set_affinity_notifier(), if there was outstanding work a ref had been got for it but was never put. Fix both by checking the return values of the work handling functions (schedule_work() for (1) and cancel_work_sync() for (2)) and put the extra ref if the return value indicates preexisting work. genirq, sched/isolation: Isolate from handling managed interrupts
The affinity of managed interrupts is completely handled in the kernel and cannot be changed via the /proc/irq/* interfaces from user space. As the kernel tries to spread out interrupts evenly accross CPUs on x86 to prevent vector exhaustion, it can happen that a managed interrupt whose affinity mask contains both isolated and housekeeping CPUs is routed to an isolated CPU. As a consequence IO submitted on a housekeeping CPU causes interrupts on the isolated CPU. Add a new sub-parameter 'managed_irq' for 'isolcpus' and the corresponding logic in the interrupt affinity selection code. The subparameter indicates to the interrupt affinity selection logic that it should try to avoid the above scenario. This isolation is best effort and only effective if the automatically assigned interrupt mask of a device queue contains isolated and housekeeping CPUs. If housekeeping CPUs are online then such interrupts are directed to the housekeeping CPU so that IO submitted on the housekeeping CPU cannot disturb the isolated CPU. If a queue's affinity mask contains only isolated CPUs then this parameter has no effect on the interrupt routing decision, though interrupts are only happening when tasks running on those isolated CPUs submit IO. IO submitted on housekeeping CPUs has no influence on those queues. If the affinity mask contains both housekeeping and isolated CPUs, but none of the contained housekeeping CPUs is online, then the interrupt is also routed to an isolated CPU. Interrupts are only delivered when one of the isolated CPUs in the affinity mask submits IO. If one of the contained housekeeping CPUs comes online, the CPU hotplug logic migrates the interrupt automatically back to the upcoming housekeeping CPU. Depending on the type of interrupt controller, this can require that at least one interrupt is delivered to the isolated CPU in order to complete the migration. Robert Hodaszi provided finally enough debug data and an excellent initial failure analysis which allowed to understand the underlying issues. This contains a change to the core interrupt management code which is required to handle this correctly for the APIC/IO_APIC. The core changes are NOOPs for most architectures except ARM64. ARM64 is not impacted by the change as confirmed by Marc Zyngier. - Newer systems allow to disable the PIT clock for power saving causing panic in the timer interrupt delivery check of the IO/APIC when the HPET timer is not enabled either. While the clock could be turned on this would cause an endless whack a mole game to chase the proper register in each affected chipset. These systems provide the relevant frequencies for TSC, CPU and the local APIC timer via CPUID and/or MSRs, which allows to avoid the PIT/HPET based calibration. As the calibration code is the only usage of the legacy timers on modern systems and is skipped anyway when the frequencies are known already, there is no point in setting up the PIT and actually checking for the interrupt delivery via IO/APIC. To achieve this on a wide variety of platforms, the CPUID/MSR based frequency readout has been made more robust, which also allowed to remove quite some workarounds which turned out to be not longer required. genirq: Add optional hardware synchronization for shutdown
free_irq() ensures that no hardware interrupt handler is executing on a
different CPU before actually releasing resources and deactivating the
interrupt completely in a domain hierarchy.
But that does not catch the case where the interrupt is on flight at the
hardware level but not yet serviced by the target CPU. That creates an
interesing race condition:

That might be harmless and just trigger a spurious interrupt warning, but
some interrupt chips might get into a wedged state.
Utilize the existing irq_get_irqchip_state() callback for the
synchronization in free_irq().
synchronize_hardirq() is not using this mechanism as it might actually
deadlock unter certain conditions, e.g. when called with interrupts
disabled and the target CPU is the one on which the synchronization is
invoked. synchronize_irq() uses it because that function cannot be called
from non preemtible contexts as it might sleep.
No functional change intended and according to Marc the existing GIC
implementations where the driver supports the callback should be able
to cope with that core change. Famous last words.

genirq: Delay deactivation in free_irq()
When interrupts are shutdown, they are immediately deactivated in the
irqdomain hierarchy. While this looks obviously correct there is a subtle
issue:
There might be an interrupt in flight when free_irq() is invoking the
shutdown. This is properly handled at the irq descriptor / primary handler
level, but the deactivation might completely disable resources which are
required to acknowledge the interrupt.
Split the shutdown code and deactivate the interrupt after synchronization
in free_irq(). Fixup all other usage sites where this is not an issue to
invoke the combined shutdown_and_deactivate() function instead.
This still might be an issue if the interrupt in flight servicing is
delayed on a remote CPU beyond the invocation of synchronize_irq(), but
that cannot be handled at that level and needs to be handled in the
synchronize_irq() context.

genirq: Prevent use-after-free and work list corruption
When irq_set_affinity_notifier() replaces the notifier, then the reference count on the old notifier is dropped which causes it to be freed. But nothing ensures that the old notifier is not longer queued in the work list. If it is queued this results in a use after free and possibly in work list corruption. Ensure that the work is canceled before the reference is dropped. genirq: Provide basic NMI management for interrupt lines
Add functionality to allocate interrupt lines that will deliver IRQs as Non-Maskable Interrupts. These allocations are only successful if the irqchip provides the necessary support and allows NMI delivery for the interrupt line. Interrupt lines allocated for NMI delivery must be enabled/disabled through enable_nmi/disable_nmi_nosync to keep their state consistent. To treat a PERCPU IRQ as NMI, the interrupt must not be shared nor threaded, the irqchip directly managing the IRQ must be the root irqchip and the irqchip cannot be behind a slow bus. genirq: Make force irq threading setup more robust
The support of force threading interrupts which are set up with both a
primary and a threaded handler wreckaged the setup of regular requested
threaded interrupts (primary handler == NULL).
The reason is that it does not check whether the primary handler is set to
the default handler which wakes the handler thread. Instead it replaces the
thread handler with the primary handler as it would do with force threaded
interrupts which have been requested via request_irq(). So both the primary
and the thread handler become the same which then triggers the warnon that
the thread handler tries to wakeup a not configured secondary thread.
Fortunately this only happens when the driver omits the IRQF_ONESHOT flag
when requesting the threaded interrupt, which is normaly caught by the
sanity checks when force irq threading is disabled.
Fix it by skipping the force threading setup when a regular threaded
interrupt is requested. As a consequence the interrupt request which lacks
the IRQ_ONESHOT flag is rejected correctly instead of silently wreckaging
it.

genirq: Synchronize only with single thread on free_irq()
When pciehp is converted to threaded IRQ handling, removal of unplugged devices below a PCIe hotplug port happens synchronously in the IRQ thread. Removal of devices typically entails a call to free_irq() by their drivers. If those devices share their IRQ with the hotplug port, __free_irq() deadlocks because it calls synchronize_irq() to wait for all hard IRQ handlers as well as all threads sharing the IRQ to finish. Actually it's sufficient to wait only for the IRQ thread of the removed device, so call synchronize_hardirq() to wait for all hard IRQ handlers to finish, but no longer for any threads. Compensate by rearranging the control flow in irq_wait_for_interrupt() such that the device's thread is allowed to run one last time after kthread_stop() has been called. kthread_stop() blocks until the IRQ thread has completed. On completion the IRQ thread clears its oneshot thread_mask bit. This is safe because __free_irq() holds the request_mutex, thereby preventing __setup_irq() from handing out the same oneshot thread_mask bit to a newly requested action.
