Instead of using cpu_is_omap..() macros in the device driver we
rely on information provided in the platform data.
The only information we need is whether the USB Host module has
a single ULPI bypass control bit for all ports or individual bypass
control bits for each port. OMAP3 REV2.1 and earlier have the former.

In MD raid case, discard granularity might not be power of 2, for example, a
4-disk raid5 has 3*chunk_size discard granularity. Correct the calculation for
such cases.

Jan Beulich points out __COUNTER__ (gcc 4.3 and above), so let's use
that to create unique ids. This is better than __LINE__ which we use
today, so provide a wrapper.
Stanislaw Gruszka <sgruszka@redhat.com> reported that some module parameters
start with a digit, so we need to prepend when we for the unique id.

With the addition of the new kernel module syscall, which defines two
arguments - a file descriptor to the kernel module and a pointer to a NULL
terminated string of module arguments - it is now possible to measure and
appraise kernel modules like any other file on the file system.
This patch adds support to measure and appraise kernel modules in an
extensible and consistent manner.
To support filesystems without extended attribute support, additional
patches could pass the signature as the first parameter.

Now that kernel module origins can be reasoned about, provide a hook to
the LSMs to make policy decisions about the module file. This will let
Chrome OS enforce that loadable kernel modules can only come from its
read-only hash-verified root filesystem. Other LSMs can, for example,
read extended attributes for signatures, etc.

As part of the effort to create a stronger boundary between root and
kernel, Chrome OS wants to be able to enforce that kernel modules are
being loaded only from our read-only crypto-hash verified (dm_verity)
root filesystem. Since the init_module syscall hands the kernel a module
as a memory blob, no reasoning about the origin of the blob can be made.
Earlier proposals for appending signatures to kernel modules would not be
useful in Chrome OS, since it would involve adding an additional set of
keys to our kernel and builds for no good reason: we already trust the
contents of our root filesystem. We don't need to verify those kernel
modules a second time. Having to do signature checking on module loading
would slow us down and be redundant. All we need to know is where a
module is coming from so we can say yes/no to loading it.
If a file descriptor is used as the source of a kernel module, many more
things can be reasoned about. In Chrome OS's case, we could enforce that
the module lives on the filesystem we expect it to live on. In the case
of IMA (or other LSMs), it would be possible, for example, to examine
extended attributes that may contain signatures over the contents of
the module.
This introduces a new syscall (on x86), similar to init_module, that has
only two arguments. The first argument is used as a file descriptor to
the module and the second argument is a pointer to the NULL terminated
string of module arguments.

We're currently offering a whopping 32 memory slots to user space, an
int is a bit excessive for storing this. We would like to increase
our memslots, but SHRT_MAX should be more than enough.

struct kvm_userspace_memory_region.flags is a u32 with a comment that
bits 0 ~ 15 are visible to userspace and the other bits are reserved
for kvm internal use. KVM_MEMSLOT_INVALID is the only internal use
flag and it has a comment that bits 16 ~ 31 are internally used and
the other bits are visible to userspace.
Therefore, let's define this as a u32 so we don't waste bytes on LP64
systems. Move to the end of the struct for alignment.

Merge misc VM changes from Andrew Morton:
"The rest of most-of-MM. The other MM bits await a slab merge.
This patch includes the addition of a huge zero_page. Not a
performance boost but it an save large amounts of physical memory in
some situations.
Also a bunch of Fujitsu engineers are working on memory hotplug.
Which, as it turns out, was badly broken. About half of their patches
are included here; the remainder are 3.8 material."
However, this merge disables CONFIG_MOVABLE_NODE, which was totally
broken. We don't add new features with "default y", nor do we add
Kconfig questions that are incomprehensible to most people without any
help text. Does the feature even make sense without compaction or
memory hotplug?

This would reset a connection with any OSD that had an outstanding
request that was taking more than N seconds. The idea was that if the
OSD was buggy, the client could compensate by resending the request.
In reality, this only served to hide server bugs, and we haven't
actually seen such a bug in quite a while. Moreover, the userspace
client code never did this.
More importantly, often the request is taking a long time because the
OSD is trying to recover, or overloaded, and killing the connection
and retrying would only make the situation worse by giving the OSD
more work to do.

Optimize RAID6 recovery functions to take advantage of
the 256-bit YMM integer instructions introduced in AVX2.
The patch was tested and benchmarked before submission.
However hardware is not yet released so benchmark numbers
cannot be reported.

Currently a zone's present_pages is calcuated as below, which is
inaccurate and may cause trouble to memory hotplug.
spanned_pages - absent_pages - memmap_pages - dma_reserve.
During fixing bugs caused by inaccurate zone->present_pages, we found
zone->present_pages has been abused. The field zone->present_pages may
have different meanings in different contexts:
1) pages existing in a zone.
2) pages managed by the buddy system.

This patchset tries to introduce a new field named "managed_pages" to
struct zone, which counts "pages managed by the buddy system". And revert
zone->present_pages to count "physical pages existing in a zone", which
also keep in consistence with pgdat->node_present_pages.
We will set an initial value for zone->managed_pages in function
free_area_init_core() and will adjust it later if the initial value is
inaccurate.
For DMA/normal zones, the initial value is set to:
(spanned_pages - absent_pages - memmap_pages - dma_reserve)
Later zone->managed_pages will be adjusted to the accurate value when the
bootmem allocator frees all free pages to the buddy system in function
free_all_bootmem_node() and free_all_bootmem().
The bootmem allocator doesn't touch highmem pages, so highmem zones'
managed_pages is set to the accurate value "spanned_pages - absent_pages"
in function free_area_init_core() and won't be updated anymore.
This patch also adds a new field "managed_pages" to /proc/zoneinfo
and sysrq showmem.

We need a node which only contains movable memory. This feature is very
important for node hotplug. If a node has normal/highmem, the memory may
be used by the kernel and can't be offlined. If the node only contains
movable memory, we can offline the memory and the node.
All are prepared, we can actually introduce N_MEMORY.
add CONFIG_MOVABLE_NODE make we can use it for movable-dedicated node

While profiling numa/core v16 with cgroup_disable=memory on the command
line, I noticed mem_cgroup_count_vm_event() still showed up as high as
0.60% in perftop.
This occurs because the function is called extremely often even when memcg
is disabled.
To fix this, inline the check for mem_cgroup_disabled() so we avoid the
unnecessary function call if memcg is disabled.
