c04dc9a6bf ARM: shark: use fixed PCI i/o mapping
Convert shark to use the fixed i/o mapping and remove io.h.

This shrinks the mapping from 256MB to 1MB, but nothing is using that much
space AFAICT.

230f984662 ARM: remove disable_fiq and arch_ret_to_user macros
Now that most platforms don't need disable_fiq and arch_ret_to_user
macros, we can remove the empty macros or empty entry-macro.S files.

2342aa282b ARM: entry: shark: don't directly reference registers in macros
Directly referencing registers in macros makes assembly code harder
to change, because the macros have side effects which are non-obvious.
Use the provided 'base' register rather than directly referencing r4.

a09e64fbc0 [ARM] Move include/asm-arm/arch-* to arch/arm/*/include/mach
This just leaves include/asm-arm/plat-* to deal with.
