Split out all the bits that are purely for dma_map_ops implementations
and related code into a new <linux/dma-map-ops.h> header so that they
don't get pulled into all the drivers. That also means the architecture
specific <asm/dma-mapping.h> is not pulled in by <linux/dma-mapping.h>
any more, which leads to a missing includes that were pulled in by the
x86 or arm versions in a few not overly portable drivers.

The vio and ibmebus buses are used for pseries specific
paravirtualised devices and currently they're initialised by the
generic initcall types. This is mostly fine, but it can result in some
nuisance errors in dmesg when booting on PowerNV on some OSes, e.g.

We don't see anything similar for the ibmebus because that depends on
!CONFIG_LITTLE_ENDIAN.
This patch squashes those by switching to using machine_*_initcall()
so the bus type is only registered when the kernel is running on a
pseries machine.

Symbolic macros are unintuitive and hard to read, whereas octal constants
are much easier to interpret. Replace macros for the basic permission
flags (user/group/other read/write/execute) with numeric constants
instead, across the whole powerpc tree.
Introducing a significant number of changes across the tree for no runtime
benefit isn't exactly desirable, but so long as these macros are still
used in the tree people will keep sending patches that add them. Not only
are they hard to parse at a glance, there are multiple ways of coming to
the same value (as you can see with 0444 and 0644 in this patch) which
hurts readability.

The modalias sysfs attr is lacking a newline for DT aliases on platform
devices. The macio and ibmebus correctly add the newline, but open code it.
Introduce a new function, of_device_modalias(), that fills the buffer with
the modalias including the newline and update users of the old
of_device_get_modalias function.

There are no ibmebus driver that make use of legacy suspend/resume. This
patch removes the support for it from ibmebus framework, new ibmebus
driver (as unlikely as they are) wanting to use suspend/resume should
use dev_pm_ops.
Since there aren't any special bus specific things to do during
suspend/resume and since the PM core will automatically fallback
directly to using the device's PM ops if no bus PM ops are specified
there is no need to have any special ibmebus PM ops at all.
