012936502a usb: mtu3: fix kernel panic at qmu transfer done irq handler
[ Upstream commit d28f4091ea7ec3510fd6a3c6d433234e7a2bef14 ]

When handle qmu transfer irq, it will unlock @mtu->lock before give back
request, if another thread handle disconnect event at the same time, and
try to disable ep, it may lock @mtu->lock and free qmu ring, then qmu
irq hanlder may get a NULL gpd, avoid the KE by checking gpd's value before
handling it.

e.g.
qmu done irq on cpu0 thread running on cpu1

qmu_done_tx()
  handle gpd [0]
    mtu3_requ_complete()        mtu3_gadget_ep_disable()
      unlock @mtu->lock
        give back request         lock @mtu->lock
                                    mtu3_ep_disable()
                                      mtu3_gpd_ring_free()
                                   unlock @mtu->lock
      lock @mtu->lock
    get next gpd [1]

[1]: goto [0] to handle next gpd, and next gpd may be NULL.

8ba02cebb7 usb: mtu3: fix USB 3.0 dual-role-switch from device to host
commit 456244aeecd54249096362a173dfe06b82a5cafa upstream.

Issue description:
  When an OTG port has been switched to device role and then switch back
  to host role again, the USB 3.0 Host (XHCI) will not be able to detect
  "plug in event of a connected USB 2.0/1.0 ((Highspeed and Fullspeed)
  devices until system reboot.

Root cause and Solution:
  There is a condition checking flag "ssusb->otg_switch.is_u3_drd" in
  toggle_opstate(). At the end of role switch procedure, toggle_opstate()
  will be called to set DC_SESSION and SOFT_CONN bit. If "is_u3_drd" was
  set and switched the role to USB host 3.0, bit DC_SESSION and SOFT_CONN
  will be skipped hence caused the port cannot detect connected USB 2.0
  (Highspeed and Fullspeed) devices. Simply remove the condition check to
  solve this issue.

b798b677f9 usb: mtu3: fix interval value for intr and isoc
[ Upstream commit e3d4621c22f90c33321ae6a6baab60cdb8e5a77c ]

Use the Interval value from isoc/intr endpoint descriptor, no need
minus one. The original code doesn't cause transfer error for
normal cases, but it may have side effect with respond time of ERDY
or tPingTimeout.

f10b01c48f usb: mtu3: add memory barrier before set GPD's HWO
commit a7aae769ca626819a7f9f078ebdc69a8a1b00c81 upstream.

There is a seldom issue that the controller access invalid address
and trigger devapc or emimpu violation. That is due to memory access
is out of order and cause gpd data is not correct.
Add mb() to prohibit compiler or cpu from reordering to make sure GPD
is fully written before setting its HWO.

62fb45d317 USB: ch9: add "USB_" prefix in front of TEST defines
For some reason, the TEST_ defines in the usb/ch9.h files did not have
the USB_ prefix on it, making it a bit confusing when reading the file,
as well as not the nicest thing to do in a uapi file.

So fix that up and add the USB_ prefix on to them, and fix up all
in-kernel usages. This included deleting the duplicate copy in the
net2272.h file.

bce3052f0c usb: roles: Provide the switch drivers handle to the switch in the API
The USB role callback functions had a parameter pointing to
the parent device (struct device) of the switch. The
assumption was that the switch parent is always the
controller. Firstly, that may not be true in every case, and
secondly, it prevents us from supporting devices that supply
multiple muxes.

Changing the first parameter of usb_role_switch_set_t and
usb_role_switch_get_t from struct device to struct
usb_role_switch.

b1a71c9047 usb: mtu3: fix race condition about delayed_status
usb_composite_setup_continue() may be called before composite_setup()
return USB_GADGET_DELAYED_STATUS, then the controller driver will
delay status stage after composite_setup() finish, but the class driver
don't ask the controller to continue delayed status anymore, this will
cause control transfer timeout.

happens when use mass storage (also enabled other class driver):

cpu1:                               cpu2
handle_setup(SET_CONFIG) //gadget driver
  unlock (g->lock)
  gadget_driver->setup()
    composite_setup()
      lock(cdev->lock)
        set_config()
          fsg_set_alt() // maybe some times due to many class are enabled
            raise FSG_STATE_CONFIG_CHANGE
            return USB_GADGET_DELAYED_STATUS
                                    handle_exception()
                                    usb_composite_setup_continue()
      unlock(cdev->lock)
                                      lock(cdev->lock)
                                        ep0_queue()
                                          lock (g->lock)
                                          //no delayed status, nothing todo
                                          unlock (g->lock)
                                      unlock(cdev->lock)
    return USB_GADGET_DELAYED_STATUS // composite_setup
  lock (g->lock)
get USB_GADGET_DELAYED_STATUS //handle_setup [1]

Try to fix the race condition as following:
After the driver gets USB_GADGET_DELAYED_STATUS at [1], if we find
there is a usb_request in ep0 request list, it means composite already
asked us to continue delayed status by usb_composite_setup_continue(),
so we skip request about delayed_status by composite_setup() and still
do status stage.
