2afd8fcee0 scsi: pm80xx: Avoid leaking tags when processing OPC_INB_SET_CONTROLLER_CONFIG command

0decc581e1 scsi: pm80xx: Use phy-specific SAS address when sending PHY_START command

Some cards have more than one SAS address. Using an incorrect address
causes communication issues with some devices like expanders.

8768583b25 scsi: pm8001: Setup IRQs on resume
commit c91774818b041ed290df29fb1dc0725be9b12e83 upstream.

The function pm8001_pci_resume() only calls pm8001_request_irq() without
calling pm8001_setup_irq(). This causes the IRQ allocation to fail, which
leads all drives being removed from the system.

Fix this issue by integrating the code for pm8001_setup_irq() directly
inside pm8001_request_irq() so that MSI-X setup is performed both during
normal initialization and resume operations.

da9cf24aa7 scsi: pm80xx: Enable upper inbound, outbound queues

Executing driver on servers with more than 32 CPUs were faced with command
timeouts. This is because we were not geting completions for commands
submitted on IQ32 - IQ63.

Set E64Q bit to enable upper inbound and outbound queues 32 to 63 in the
MPI main configuration table.

Added 500ms delay after successful MPI initialization as mentioned in
controller datasheet.

e08d269712 scsi: pm80xx: Mask and unmask upper interrupt vectors 32-63

When upper inbound and outbound queues 32-63 are enabled, we see upper
vectors 32-63 in interrupt service routine. We need corresponding registers
to handle masking and unmasking of these upper interrupts.

To achieve this, we use registers MSGU_ODMR_U(0x34) to mask and
MSGU_ODMR_CLR_U(0x3C) to unmask the interrupts. In these registers bit 0-31
represents interrupt vectors 32-63.

In pm8001_chip_set_dev_state_req(), pm8001_chip_fw_flash_update_req(),
pm80xx_chip_phy_ctl_req() and pm8001_chip_reg_dev_req() add missing calls
to pm8001_tag_free() to free the allocated tag when pm8001_mpi_build_cmd()
fails.

Similarly, in pm8001_exec_internal_task_abort(), if the chip ->task_abort
method fails, the tag allocated for the abort request task must be
freed. Add the missing call to pm8001_tag_free().

3bd9a28798 scsi: pm8001: Fix pm8001_mpi_task_abort_resp()

The call to pm8001_ccb_task_free() at the end of
pm8001_mpi_task_abort_resp() already frees the ccb tag. So when the device
NCQ_ABORT_ALL_FLAG is set, the tag should not be freed again.  Also change
the hardcoded 0xBFFFFFFF value to ~NCQ_ABORT_ALL_FLAG as it ought to be.

ef969095c4 scsi: pm8001: Fix pm80xx_pci_mem_copy() interface

The declaration of the local variable destination1 in pm80xx_pci_mem_copy()
as a pointer to a u32 results in the sparse warning:

warning: incorrect type in assignment (different base types)
    expected unsigned int [usertype]
    got restricted __le32 [usertype]

Furthermore, the destination" argument of pm80xx_pci_mem_copy() is wrongly
declared with the const attribute.

Fix both problems by changing the type of the "destination" argument to
"__le32 *" and use this argument directly inside the pm80xx_pci_mem_copy()
function, thus removing the need for the destination1 local variable.

In pm80xx_send_abort_all(), the n_elem field of the ccb used is not
initialized to 0. This missing initialization sometimes lead to the task
completion path seeing the ccb with a non-zero n_elem resulting in the
execution of invalid dma_unmap_sg() calls in pm8001_ccb_task_free(),

780c668a2d scsi: pm8001: Fix NCQ NON DATA command completion handling

NCQ NON DATA is an NCQ command with the DMA_NONE DMA direction and so a
register-device-to-host-FIS response is expected for it.

However, for an IO_SUCCESS case, mpi_sata_completion() expects a
set-device-bits-FIS for any ata task with an use_ncq field true, which
includes NCQ NON DATA commands.

Fix this to correctly treat NCQ NON DATA commands as non-data by also
testing for the DMA_NONE DMA direction.

f7a3f9e4e8 scsi: pm8001: Fix NCQ NON DATA command task initialization

In the pm8001_chip_sata_req() and pm80xx_chip_sata_req() functions, all
tasks with a DMA direction of DMA_NONE (no data transfer) are initialized
using the ATAP value 0x04. However, NCQ NON DATA commands, while being
DMA_NONE commands are NCQ commands and need to be initialized using the
value 0x07 for ATAP, similarly to other NCQ commands.

Make sure that NCQ NON DATA command tasks are initialized similarly to
other NCQ commands by also testing the task "use_ncq" field in addition to
the DMA direction. While at it, reorganize the code into a chain of if -
else if - else to avoid useless affectations and debug messages.

Make sure that the __le32 fields of struct sata_cmd are manipulated after
applying the correct endian conversion. That is, use cpu_to_le32() for
assigning values and le32_to_cpu() for consulting a field value.  In
particular, make sure that the calculations for the 4G boundary check are
done using CPU endianness and *not* little endian values. With these fixes,
many sparse warnings are removed.

While at it, fix some code identation and add blank lines after variable
declarations and in some other places to make this code more readable.

All fields of the SASProtocolTimerConfig structure have the __le32 type.
As such, use cpu_to_le32() to initialize them. This change suppresses many
sparse warnings:

warning: incorrect type in assignment (different base types)
   expected restricted __le32 [addressable] [usertype] pageCode
   got int

Note that the check to limit the value of the STP_IDLE_TMO field is removed
as this field is initialized using the fixed (and small) value defined by
the STP_IDLE_TIME macro.

The pm8001_dbg() calls printing the values of the SASProtocolTimerConfig
structure fileds are changed to use le32_to_cpu() to present the values in
human readable form.

Since the sata_cmd struct is zeroed out before its fields are initialized,
there is no need for using "|=" to initialize the ncqtag_atap_dir_m
field. Using a standard assignment removes the sparse warning:

Currently a use-after-free may occur if a sas_task is aborted by the upper
layer before we handle the I/O completion in mpi_ssp_completion() or
mpi_sata_completion().

In this case, the following are the two steps in handling those I/O
completions:

 - Call complete() to inform the upper layer handler of completion of
   the I/O.

 - Release driver resources associated with the sas_task in
   pm8001_ccb_task_free() call.

When complete() is called, the upper layer may free the sas_task. As such,
we should not touch the associated sas_task afterwards, but we do so in the
pm8001_ccb_task_free() call.

Fix by swapping the complete() and pm8001_ccb_task_free() calls ordering.

d872e7b5fe scsi: pm8001: Fix use-after-free for aborted TMF sas_task

Currently a use-after-free may occur if a TMF sas_task is aborted before we
handle the IO completion in mpi_ssp_completion(). The abort occurs due to
timeout.

When the timeout occurs, the SAS_TASK_STATE_ABORTED flag is set and the
sas_task is freed in pm8001_exec_internal_tmf_task().

However, if the I/O completion occurs later, the I/O completion still
thinks that the sas_task is available. Fix this by clearing the ccb->task
if the TMF times out - the I/O completion handler does nothing if this
pointer is cleared.

12cf120803 scsi: pm8001: Fix bogus FW crash for maxcpus=1

According to the comment in check_fw_ready() we should not check the
IOP1_READY field in register SCRATCH_PAD_1 for 8008 or 8009 controllers.

However we check this very field in process_oq() for processing the highest
index interrupt vector. The highest interrupt vector is checked as the FW
is programmed to signal fatal errors through this irq.

Change that function to not check IOP1_READY for those mentioned
controllers, but do check ILA_READY in both cases.

The reason I assume that this was not hit earlier was because we always
allocated 64 MSI(X), and just did not pass the vector index check in
process_oq(), i.e.  the handler never ran for vector index 63.

After a REPORT LUN command issued to the offline device fails, the SCSI
midlayer tries to do a sequential scan of all devices whose LUN number is
not 0. However, SATA does not support LUN numbers at all.

Introduce a generic sas_slave_alloc() handler which will return -ENXIO for
SATA devices if the requested LUN number is larger than 0 and make libsas
drivers use this function as their .slave_alloc callback.

8eaa762e1d scsi: pm80xx: Fix drives missing during rmmod/insmod loop

When driver is loaded after rmmod some drives are not showing up during
discovery.

SATA drives are directly attached to the controller connected phys.  During
device discovery, the IDENTIFY command (qc timeout (cmd 0xec)) is timing out
during revalidation. This will trigger abort from host side and controller
successfully aborts the command and returns success. Post this successful
abort response ATA library decides to mark the disk as NODEV.

To overcome this, inside pm8001_scan_start() after phy_start() call, add get
start response and wait for few milliseconds to trigger next phy start.
This millisecond delay will give sufficient time for the controller state
machine to accept next phy start.

The mpi_uninit_check() takes longer for inbound doorbell register to be
cleared. Increase the timeout substantially so that the driver does not
fail to load.

Previously, the inbound doorbell wait time was mistakenly increased in the

mpi_init_check() wait time as-is as these are timeout values and if there
is a failure, waiting longer is not an issue.

LLDDs report events to libsas with .notify_port_event and .notify_phy_event
callbacks.

These callbacks are fixed and so there is no reason why the functions
cannot be called directly, so do that.

This neatens the code slightly, makes it more obvious, and reduces function
pointer usage, which is generally a good thing. Downside is that there are
2x more symbol exports.

Every PM8001_<FOO>_DBG macro uses an internal call to pm8001_printk.

Convert all uses of:

	PM8001_<FOO>_DBG(hba, pm8001_printk(fmt, ...))
to
	pm8001_dbg(hba, <FOO>, fmt, ...)

so the visual complexity of each macro is reduced.

The repetitive macro definitions are converted to a single pm8001_dbg and
the level is concatenated using PM8001_##level##_LOGGING for the specific
level test.

Done with coccinelle, checkpatch and a little typing of the new macro
definition.

A use-after-free or null-pointer error occurs when the 251-byte response
data is copied from IOMB buffer to response message buffer in function
pm8001_mpi_get_nvmd_resp().

After sending the command get_nvmd_data(), the caller begins to sleep by
calling wait_for_complete() and waits for the wake-up from calling
complete() in pm8001_mpi_get_nvmd_resp(). Due to unexpected events (e.g.,
interrupt), if response buffer gets freed before memcpy(), a use-after-free
error will occur. To fix this, the complete() should be called after
memcpy().

3e4b377074 scsi: pm80xx: Make running_req atomic

Incorrect value of the running_req was causing the driver unload to be
stuck during the SAS lldd_dev_gone notification handling.  During SATA I/O
completion, for some error status values, the driver schedules the event
handler and running_req is decremented from that.  However, there are some
other error status values (like IO_DS_IN_RECOVERY,
IO_XFER_ERR_LAST_PIO_DATAIN_CRC_ERR) where the I/O has already been
completed by fw/driver so running_req is not decremented.

Also during NCQ error handling, driver itself will initiate READ_LOG_EXT
and ABORT_ALL. When libsas/libata initiate READ_LOG_EXT (0x2F), driver
increments running_req. This will be completed by the driver in
pm80xx_chip_sata_req(), but running_req was not decremented.

Driver submits all internal requests (like abort_task, event acknowledgment
etc.) through inbound queue 0. While submitting those, driver does not
acquire any lock and this may lead to a race when there is an I/O request
coming in on CPU0 and submitted through inbound queue 0.  To avoid this,
lock acquisition has been moved to pm8001_mpi_build_cmd().  All command
submission will go through this path.

5a141315ed scsi: pm80xx: Increase the number of outstanding I/O supported to 1024
The pm80xx driver currently sets the controller queue depth to
256. Hoewver, the controller supports outstanding I/Os up 1024.

Increase the number of outstanding I/Os from 256 to 1024.  CCBs and tags
are allocated according to outstanding I/Os. Also update the can_queue
value (max_out_io - PM8001_RESERVE_SLOT) used by the SCSI midlayer.

27bc43bd7c scsi: pm80xx: Remove DMA memory allocation for ccb and device structures
Remove DMA memory allocation for Devices and CCB structure. Instead
allocate memory outside of DMA memory.  DMA memory is a limited system
resource and it is better to allocate memory outside of DMA memory when
possible.

05c6c029a4 scsi: pm80xx: Increase number of supported queues
Current driver uses fixed number of Inbound and Outbound queues and all of
the I/O, TMF and internal requests are submitted through those. A global
spin lock is used to control the shared access. This can create a lock
contention and it is real bottleneck in the I/O path.

To avoid this, the number of supported Inbound and Outbound queues is
increased to 64, and the number of queues used is decided based on number
of CPU cores online and number of MSI-X vectors allocated. Also add locks
per queue instead of using the global lock.
