2bdd081a7d bnx2x: fix page fault following EEH recovery

In the last step of the EEH recovery process, the EEH driver calls into
bnx2x_io_resume() to re-initialize the NIC hardware via the function
bnx2x_nic_load().  If an error occurs during bnx2x_nic_load(), OS and
hardware resources are released and an error code is returned to the
caller.  When called from bnx2x_io_resume(), the return code is ignored
and the network interface is brought up unconditionally.  Later attempts
to send a packet via this interface result in a page fault due to a null
pointer reference.

This patch checks the return code of bnx2x_nic_load(), prints an error
message if necessary, and does not enable the interface.

c40b76dfa7 bnx2x: fix pci device refcount leak in bnx2x_vf_is_pcie_pending()

As comment of pci_get_domain_bus_and_slot() says, it returns
a pci device with refcount increment, when finish using it,
the caller must decrement the reference count by calling
pci_dev_put(). Call pci_dev_put() before returning from
bnx2x_vf_is_pcie_pending() to avoid refcount leak.

6cc0e2afc6 bnx2x: fix potential memory leak in bnx2x_tpa_stop()

bnx2x_tpa_stop() allocates a memory chunk from new_data with
bnx2x_frag_alloc(). The new_data should be freed when gets some error.
But when "pad + len > fp->rx_buf_size" is true, bnx2x_tpa_stop() returns
without releasing the new_data, which will lead to a memory leak.

We should free the new_data with bnx2x_frag_free() when "pad + len >
fp->rx_buf_size" is true.

9591967ac4 bnx2x: fix napi API usage sequence

While handling PCI errors (AER flow) driver tries to
disable NAPI [napi_disable()] after NAPI is deleted
[__netif_napi_del()] which causes unexpected system
hang/crash.

EEH calls into bnx2x twice based on the system log above, first through

bf5b7aae86 bnx2x: fix built-in kernel driver load failure

Commit b7a49f73059f ("bnx2x: Utilize firmware 7.13.21.0")
added request_firmware() logic in probe() which caused
load failure when firmware file is not present in initrd (below),
as access to firmware file is not feasible during probe.

  Direct firmware load for bnx2x/bnx2x-e2-7.13.15.0.fw failed with error -2
  Direct firmware load for bnx2x/bnx2x-e2-7.13.21.0.fw failed with error -2

This patch fixes this issue by -

1. Removing request_firmware() logic from the probe()
   such that .ndo_open() handle it as it used to handle
   it earlier

2. Given request_firmware() is removed from probe(), so
   driver has to relax FW version comparisons a bit against
   the already loaded FW version (by some other PFs of same
   adapter) to allow different compatible/close enough FWs with which
   multiple PFs may run with (in different environments), as the
   given PF who is in probe flow has no idea now with which firmware
   file version it is going to initialize the device in ndo_open()

7e1eae5d1a bnx2x: fix driver load from initrd
commit e13ad1443684f7afaff24cf207e85e97885256bd upstream.

Commit b7a49f73059f ("bnx2x: Utilize firmware 7.13.21.0") added
new firmware support in the driver with maintaining older firmware
compatibility. However, older firmware was not added in MODULE_FIRMWARE()
which caused missing firmware files in initrd image leading to driver load
failure from initrd. This patch adds MODULE_FIRMWARE() for older firmware
version to have firmware files included in initrd.

4d63363c88 bnx2x: Invalidate fastpath HSI version for VFs
commit 802d4d207e75d7208ff75adb712b556c1e91cf1c upstream

Commit 0a6890b9b4 ("bnx2x: Utilize FW 7.13.15.0.")
added validation for fastpath HSI versions for different
client init which was not meant for SR-IOV VF clients, which
resulted in firmware asserts when running VF clients with
different fastpath HSI version.

This patch along with the new firmware support in patch #1
fixes this behavior in order to not validate fastpath HSI
version for the VFs.

fdcfabd095 bnx2x: Utilize firmware 7.13.21.0
commit b7a49f73059fe6147b6b78e8f674ce0d21237432 upstream

This new firmware addresses few important issues and enhancements
as mentioned below -

- Support direct invalidation of FP HSI Ver per function ID, required for
  invalidating FP HSI Ver prior to each VF start, as there is no VF start
- BRB hardware block parity error detection support for the driver
- Fix the FCOE underrun flow
- Fix PSOD during FCoE BFS over the NIC ports after preboot driver
- Maintains backward compatibility

This patch incorporates this new firmware 7.13.21.0 in bnx2x driver.

d0ea5cbdc2 drivers/net/ethernet: clean up mis-targeted comments
As part of the W=1 cleanups for ethernet, a million [*] driver
comments had to be cleaned up to get the W=1 compilation to
succeed. This change finally makes the drivers/net/ethernet tree
compile with W=1 set on the command line. NOTE: The kernel uses
kdoc style (see Documentation/process/kernel-doc.rst) when
documenting code, not doxygen or other styles.

After this patch the x86_64 build has no warnings from W=1, however
scripts/kernel-doc says there are 1545 more warnings in source files, that
I need to develop a script to fix in a followup patch.

The errors fixed here are all kdoc of a few classes, with a few outliers:

5198d545db net: remove napi_hash_del() from driver-facing API
We allow drivers to call napi_hash_del() before calling
netif_napi_del() to batch RCU grace periods. This makes
the API asymmetric and leaks internal implementation details.
Soon we will want the grace period to protect more than just
the NAPI hash table.

Restructure the API and have drivers call a new function -
__netif_napi_del() if they want to take care of RCU waits.

Note that only core was checking the return status from
napi_hash_del() so the new helper does not report if the
NAPI was actually deleted.

Some notes on driver oddness:
 - veth observed the grace period before calling netif_napi_del()
   but that should not matter
 - myri10ge observed normal RCU flavor
 - bnx2x and enic did not actually observe the grace period
   (unless they did so implicitly)
 - virtio_net and enic only unhashed Rx NAPIs

The last two points seem to indicate that the calls to
napi_hash_del() were a left over rather than an optimization.
Regardless, it's easy enough to correct them.

This patch may introduce extra synchronize_net() calls for
interfaces which set NAPI_STATE_NO_BUSY_POLL and depend on
free_netdev() to call netif_napi_del(). This seems inevitable
since we want to use RCU for netpoll dev->napi_list traversal,
and almost no drivers set IFF_DISABLE_NETPOLL.

4ced637bd2 bnx2x: use generic power management
With legacy PM, drivers themselves were responsible for managing the
device's power states and takes care of register states.

After upgrading to the generic structure, PCI core will take care of
required tasks and drivers should do only device-specific operations.

The driver was also calling bnx2x_set_power_state() to set the power state
of the device by changing the device's registers' value. It is no more
needed.

Compile-tested only.

bnx2x_warpcore_read_sfp_module_eeprom() can call bnx2x_bsc_read()
three times before giving up.

This causes latency blips of at least 31 ms (58 ms being reported
by our teams)

Convert the long lasting loops of udelay() to usleep_range() ones,
and breaks the loops on precise time tracking.

dd3e7cba16 ocfs2/dlm: move BITS_TO_BYTES() to bitops.h for wider use
There are users already and will be more of BITS_TO_BYTES() macro.  Move
it to bitops.h for wider use.

In the case of ocfs2 the replacement is identical.

As for bnx2x, there are two places where floor version is used.  In the
first case to calculate the amount of structures that can fit one memory
page.  In this case obviously the ceiling variant is correct and
original code might have a potential bug, if amount of bits % 8 is not
0.  In the second case the macro is used to calculate bytes transmitted
in one microsecond.  This will work for all speeds which is multiply of
1Gbps without any change, for the rest new code will give ceiling value,
for instance 100Mbps will give 13 bytes, while old code gives 12 bytes
and the arithmetically correct one is 12.5 bytes.  Further the value is
used to setup timer threshold which in any case has its own margins due
to certain resolution.  I don't see here an issue with slightly shifting
thresholds for low speed connections, the card is supposed to utilize
highest available rate, which is usually 10Gbps.

5cdc40c782 bnx2x: Fix accounting of vlan resources among the PFs
While testing max vlan configuration on the PF, firmware gets
assert as driver was configuring number of vlans more than what
is supported per port/engine, it was figured out that there is an
implicit vlan (hidden default vlan consuming hardware cam entry resource)
which is configured default for all the clients (PF/VFs) on client_init
ramrod by the adapter implicitly, so when allocating resources among the
PFs this implicit vlan should be considered or total vlan entries should
be reduced by one to accommodate that default/implicit vlan entry.

ee699f89bd bnx2x: Fix logic to get total no. of PFs per engine
Driver doesn't calculate total number of PFs configured on a
given engine correctly which messed up resources in the PFs
loaded on that engine, leading driver to exceed configuration
of resources (like vlan filters etc.) beyond the limit per
engine, which ended up with asserts from the firmware.

7113f796bb bnx2x: Do not handle requests from VFs after parity
Parity error from the hardware will cause PF to lose the state
of their VFs due to PF's internal reload and hardware reset following
the parity error. Restrict any configuration request from the VFs after
the parity as it could cause unexpected hardware behavior, only way
for VFs to recover would be to trigger FLR on VFs and reload them.

548e5ffe2e bnx2x: Remove hw_reset_t function casts
All .rw_reset callbacks except bnx2x_84833_hw_reset_phy() use a
void return type. No callers of .hw_reset check a return value and
bnx2x_84833_hw_reset_phy() unconditionally returns 0. Remove all
hw_reset_t casts and fix the return type to void.

26658f6bdb bnx2x: Remove format_fw_ver_t function casts
The return values for format_fw_ver_t callbacks are supposed to be
"int", not "u8". Ultimately, the top-level caller doesn't actually check
the return value at all, but just clean this all up anyway and fix the
prototypes so that casts are no longer needed.

2c855d73f2 bnx2x: Remove read_status_t function casts
The function casts for .read_status callbacks end up casting some int
return values to u8. This seems to be bug-prone (-EINVAL being returned
into something that appears to be true/false), but fixing the function
prototypes doesn't change the existing behavior. Fix the return values
to remove the casts.

86c1fe8857 bnx2x: Drop redundant callback function casts
NULL is already "void *" so it will auto-cast in assignments and
initializers. Additionally, all the callbacks for .link_reset,
.config_loopback, .set_link_led, and .phy_specific_func are already
correct. No casting is needed for these, so remove them.
