sh: Proper __put_user_asm() size mismatch fix.
This fixes up the workaround in 2b4b2bb421
and cleans up __put_user_asm() to get the sizing right from the onset.

sh: migrate to arch/sh/include/
This follows the sparc changes a439fe51a1.
Most of the moving about was done with Sam's directions at:

with subsequent hacking and fixups entirely my fault.
