[SCSI] libfc, libfcoe: FDISC ELS for NPIV

Browse Source 
Add FDISC ELS handling to libfc and libfcoe, treat it the same as FLOGI where
appropriate.

Add checking for NPIV support in the FLOGI LS_ACC service parameters.
