tracing: Add hist trigger 'sym' and 'sym-offset' modifiers

Allow users to have address fields displayed as symbols in the output
by appending '.sym' or 'sym-offset' to field names:

   # echo hist:keys=aaa.sym,bbb.sym-offset ... \
              [ if filter] > event/trigger
