x86/paravirt: Remove no longer used paravirt functions

Browse Source 
With removal of lguest some of the paravirt functions are no longer
needed:

	->read_cr4()
	->store_idt()
	->set_pmd_at()
	->set_pud_at()
	->pte_update()

Remove them.
