[PATCH] USB: move hardware-specific <linux/usb_*.h> to <linux/usb/*.h>

Browse Source 
This moves header files for controller-specific platform data
from <linux/usb_XXX.h> to <linux/usb/XXX.h> to start reducing
some clutter.
